On 03/08/01 at 12:44 A.M., officers responded to a traffic accident on SR-57 in the area of the Midway Mall. Once on scene, they found two vehicles had collided in the northbound lanes and it appeared as if the accident had occurred as a head on collision. The two vehicles located in the northbound lanes were the above identified vehicles #2 and #3.

Officers investigated and learned the following: Vehicle #1 was southbound on SR-57. Vehicle #2 was also southbound on SR-57 and approaching Vehicle #1 from the rear. Vehicle #2 struck the rear of Vehicle #1 and then went out of control. Vehicle #2 crossed the median from the southbound lanes and into the northbound lanes. Vehicle #3 was northbound on SR-57 and was struck approximately head on by Vehicle #2 as it entered the northbound lanes. Driver of Vehicle #2 and driver of Vehicle #3 both died as a result of injuries suffered in the crash. Driver of Vehicle #1 was not injured and his vehicle suffered minor damage. The passenger in Vehicle #3 was transported to EMH Regional Medical Center and later life flighted to Cleveland Metro Medical Center.
